New Member Information Form
- Clubs may use this form to gather information for new members. This form is NOT needed in the Extension Office. All the information we need regarding a new member we can get from their Enrollment Form.

- Begin keeping this record your first year of 4-H club work.

Across County Lines
- This form must be completed by all youth who reside in another county and want to be a member of the Mercer County 4-H program. Please be aware that Ohio 4-H rules state that “any change motivated by perceived competitive or sale advantage, any change due to controversial issues, or other similar motivations will be automatically rejected.” If the request for membership across county lines is not approved, you will need to consider membership in your county of residence.

- This award is for first, second, and third year 4-H members who are 9 years of age by January 1 of the current year and not over 14 years of age. Members 8 years and in the third grade are also eligible.

Contest Form - Press Release Scrapbook
- The purpose of the scrapbook contest is to encourage 4-H clubs to SUBMIT articles to newspapers, church bulletins, etc., to promote 4-H and to make the general public more aware of all the work and activities 4-H members are doing.

Contest Form - Club History Scrapbook
- The purpose of the club history scrapbook contest is to encourage 4-H clubs to create a permanent club record. The winning entry will be judged on the completion of all areas mentioned in the score sheet.

- GENERAL GUIDELINES: The purpose of the contest is to encourage 4-H secretaries to learn how to properly take minutes and to enter those minutes into the record book AFTER each meeting.

Photo Release for a Minor
- It is recommended that a release be obtained when photographing or videotaping a minor (under 18). Parent or guardian signatures are required; signatures of minors are not sufficient.
